u-boot-dey: ccmp25: fix boot script to correctly parse HWID 

In CCMP2 the HWID is stored in 3 consecutive fuse words, now the third word has
the following scheme:

  | 31..18 | 17 |   16  |15..12|  11..7  |6..3| 2..0 |
  +--------+----+-------+------+---------+----+------+
